The "5 Whys" is a root cause analysis. Keep asking "Why?" until you find the real problem. Toyota developed it to go beyond surface-level symptoms.

Find the root cause, not just the symptoms. Understand cause-and-effect better. Create lasting solutions. Align the team on the problem.
Identified root causes. Deep problem understanding. Underlying issues addressed.
Don't blame individuals. Focus on the system. Keep asking "Why?" even if it feels repetitive. Involve people who know the problem well. Sometimes, you might not need all five 'whys' to get to the root cause. Be flexible.

  1. Define the problem (5 min). Write down the problem you're analyzing.

  2. Ask the first why (5 min). Ask 'Why did this problem occur?' Write down the answer.

  3. Continue asking why (20 min). For each answer, ask 'Why?' again. Do this five times, or until you find the root cause.

  4. Validate the logic (10 min). Go backwards from the root cause. Does the chain make sense?

  5. Develop solutions (5 min). Brainstorm solutions for the root cause.
